2.) YOU NEED TO FIRST SIGN UP (PASSWORD CAN NOT HAVE ANY
SPECIAL CHARACTERS FOR EXAMPLE.

3.) THEN YOU WILL RECEIVE AN ACTIVATION E-MAIL FOR YOUR
PATIENT PORTAL ACCOUNT.
4.) YOU WILL THEN SIGN BACK IN AND BE ABLE TO COMPLETE YOUR
HISTORY.
